A fractional CO2 laser works by splitting the 10600nm beam into a lattice of tiny micro-beams that ablate microscopic columns of tissue while leaving the skin around each column intact. The main absorption chromophore at 10600nm is water, which is also the primary component of the skin, so the laser energy converts into a precise, controlled thermal effect exactly where it is delivered.
After being applied to the skin, the lattice array of small beams produces multiple tiny three-dimensional, cylindrical thermal damage zones surrounded by normal, undamaged tissue. Because only a fraction of the surface is ablated (about 6.25% to 25% coverage depending on the selected spot density), the corneocytes from the surrounding tissue migrate quickly to repair the damaged zones and make them heal rapidly. At the same time, the heat rearranges the proliferation of collagen fiber and elastic fiber, brings the proportion of type I and type III collagen fiber close to normal, changes the tissue of pathological scar, softens the scar and gradually restores its elasticity. Dermal collagen fibers are heated so that they contract and remodel, collagen deposits in an orderly way and new collagen proliferates - improving skin elasticity and reducing scars.
